I phoned the OFAH, believe the extension is 260, and had the DVD mailed to my house, cost was $35 per person, comes with a test permission slip required to challenge the exam. No other cost involved, exam is free, call the local rep, I did the exam at his kitchen table, 20 minutes. Saves sitting in the classroom for 6 hour course
